Have you spoken the the rewinder, did they rewind as it was originally?
Is it 6 wire single speed?
Why did it need to be rewound, have the contactors or contacts been replaced?
 
Can you post a picture of the nameplate on the motor, if it can be done safely, a pic of the motor wiring terminals would be good to.
 
When a motor fails you need to establish the root cause, it is quite possible the motor failed because of a control issue so regardless of the rewinding been done correctly you may be subjecting it to the same issue that saw the original demise.
Have you done any testing to confirm voltages etc off and on load, also what is the starting method and what is the nature of the motor.
As above the nameplate will give us a start.
