Really quick one- If I install 4kW for a customer now but have a DNO request in for 8kW, can I install the 4kW under 15.44 and later on the additional 4kW under 13.99p?

you should be able to, though I'd think you want to leave it a while before installing the 2nd system or someone might think you were pulling a fast one.

we've done it either side of a FIT change before now, and checked with Ofgem that is was definitely ok and got a yes, so installed 10kW then, another 20kW a couple of months later once we had G59 approval.

I do think it's a bit of a grey area though, and I'd say as a minimum you'd want to wait until the first fit application has been accepted and fully registered before even thinking about phase 2.
