The heartbroken family of an engineer killed while working on a travelator at Waterloo station have said they hope lessons will be learned following the tragedy.Christian Tuvi, 44, died in the early hours of Wednesday, September 18 while repairing a walkway at the London Underground interchange.

Begs the question why LOTO wasn't followed? I hope for his families sake it was either not provisioned for or that the investigation finds the employer failing thier safe system of work responsibilities under HASAWA
