Strange - if it had been modified to be used with 12V then why would it still have a 13A mains plug on it which could be plugged in to a mains outlet?

We can only guess, but the reference to chrome rods connecting 'base to bulb' suggests it was one of those lamps with ELV via accessible parts such as the telescopic stand. If someone had put a mains plug on that, the rods would have been live with mains and he most likely got a shock between them (i.e. between L & N) possibly hand to hand, which would not have tripped the RCD.

Perhaps the lamp was originally 12V via transformer, someone removed the transformer to connect to the 12V supply, then someone else put a 13A plug on when mains was available. News items cannot generally be trusted to convey enough accurate information to understand the specific configuration of hardware involved in an incident.
