In order to meet current regulation the maximum meter tail length should be no more than 3M and if over a 100amp fused switch should be fitted - is that correct.
(This guidline is not in the iet on-site guide except that this maybe specified by the distributor)

521.10.1 mentions that the tails should be insulated and sheathed or insulated and enclosed within containment. What should the composition of the sheathing be, 25mm cable comes with a grey plastic sheath.
eg:
BASEC approved PVC insulated and sheathed, single core stranded cable to BS6004 Table7. Commonly used as meter tails for connecting consumer units to the electricity meter.

This cable does not require enclosure - is that correct.
 
If over 3m in length then a fuse switch is generally required. Tail lengths can then be extended as required. ...Think about tail lengths required in apartment blocks etc, they are going to be well in excess of 3m aren't they!!!

There is no definite length stipulated in either the Regs, or the OSG, the OSG as you say advises that you ask the DNO and if you exceed their recommended distance then yes you have to supply protection for the tails, normally as you say a Switch Fuse.
It may also need RCD protection as well depending on your erection methods and type of earthing system
