Does a Minor Works Certificate also apply for three phase? as under the essential test's section, it doesnt mention anything about L-L Insulation resistance test!.

A minor works is defined as any electrical work that does not include the provision of a new circuit. Therefore yes it does apply to 3 phase.

The forms in BS7671 are model forms and therefore do not include all of the result of tests that you may have carried out. For example the test results schedule does not include r1,rn and r2 boxes for the ring final circuit tests whereas the NICEIC forms which are based on the model forms in BS7671 do.
