There is a book available from the NICEIC on Inspection and Testing and filling out their forms.

ITC - Inspection, Testing andÂ*Certification 17th Edition | NICEIC Direct

1. Distributors cut-out fuse either BS1361 or BS88. Max KA will either be 33KA or 16.5KA depending on type 1 or 2.

2.All depends on the circuit in question

3.BS 7671 2008 has not been ammended yet so N/A.

4. It will either be the Main Switch in your consumer unit or if you have a sub-main to yout DB from the main intake then the rating of the Sub-main isolator needs to be recorded also.
