Under what set of circumstances would the 'other' box be ticked in the section of a certificate headed 'Number and Type of Live Conductors'? Does this have to be specified on any other part pf the paperwork?
 
I have seen a 2 phase and Neutral supply once. i ticked the box then.

Thats exactly what im working on at the mo:

The old "twin phase" was common in churches a long time ago, one phase was used for power and lighting while the other was solely for the electric heating due it being tubular heaters under all the pews.
