Also which assessor have you got ?
Somebody on the forum will know him or her and be able to put your mind at rest about how they are on assessments, remember though if you are missing a document they wont 'fail' you, they will say what they need and ask you to send it across by email afterwards.

The H&S document is on the NICEIC website, just put your name in all the applicable boxes.
Have used the same one every year for mine. I did it on computer and just go in and change the dates!
All the documents requested are on this page NICEIC Contractor - http://www.niceic.com/contractor/essentials/downloads
I am confused about section on page 6 * Employee repersntative are.... I am only one in my company.
also * Consultaion with emploees is provided by ,.........

From my experience, they really aren't out to catch you out - if only because they want your money!

As long as you can show that you can work safely and haven't made any dangerous howlers, then if anything isn't right with the paperwork, the worst that will likely happen is that you'll have to submit it in due course as a minor issue.

I've used the same standard H&S document from that page since I started 14ish years ago - noone has looked past the first page yet!
