when putting in a wall mounted fan in kitchen does it have to be 5in? or is 4in ok? for say rewire? and new build? I am pretty sure 4in is fine in wc/bathroom/utility etc- but not 100pc sure with kitchen?

It would be any fan capable of the required extract rate. Duct size does come into it but not always, especially when vented straight through a wall with no appreciable duct length. For instance an expelair DX400 4" centrifugal fan is capable of 245m3/hr and may be more appropriate over a longer duct length. Axial fans generally are not capable of shoving air more than a couple of metres.
 
i use a 5" core drill for cooker hoods of an extractor is being provided. If not 6" fan ceiling or wall mounted, for HA, Social and the likes. There is no need to provide external extraction for cooker hood if fan is in place.

to meet 216m3/h and what core cutter they use.

all fans I am finding big enough to do 216m3/h need 170mm holes cut- and struggling to find a 170mm core cutter?

I would say that 170mm would only be required if you were using thick walled 6" rigid waste pipe as a duct. Flexible ducts require a smaller hole in the more commonly available core cutter sizes. 6" fans are generally standardised as having 150mm spigots.
