:D hadn't read the footnotes, just rebuild the house :D - mind it confirms that it is vertical depth as if the holes were max 1/4 of the thickness of the joist you would barely squeeze a cable through.
 
I imagine they really mean lower the ceiling or raise the floor by the required amount. Seems reasonable and I'm sure the client would be delighted.

At the end of the day, and as usual, this will come down to a local judgement by the contractor.
 
if you can drill 0.25 times depth you can drill about a 50mm hole in a first floor joist they are about 240mm deep
