Been looking into these batteries from dewalt standard 18v battery but has an output equivalent to 54v.

It's not 'an output equivalent to 54v', it is 54V.

The cell groups re-configure from parallel to series, depending on what tool they're fitted to. (3 x 18 = 54) The total energy capacity remains the same. So 18V, 6Ah or 54V, 2Ah.
 
Yep they are just a higher capacity battery.

Unless you are going to start collecting the flexi volt range don't spend the extra.
