You don't need a sponsor or references if you're self employed.
Leave those parts blank, or write self employed accross them.

Perhaps they did for you, they didn't for me.Yes , and i also was self-employed at the time i recieved my card.
But unless you've had an indentured apprenticeship , they'll want references.
