When should an SPD be installed?

There has been a lot of confusion over when an SPD should be installed.

Should an SPD be installed if:-
1.Changing over faceplates?
2. Changing a DB?
3. Adding a point to an existing circuit?
4. Adding a completely new circuit?

And what if the client does not want to pay for an SPD?
 
This is off the top of my head without looking through the regs so I apologise if anything I say is wrong but following the amendment 2 introduction I believe the following to be correct.
Should an SPD be installed if:-

3. Adding a point to an existing circuit?
4. Adding a completely new circuit?

1. No need for spd
2. Yes an spd shall be installed in all installations. Unless within a domestic property the occupant provides written confirmation that they are aware and take full responsibility of the risks and consequences not installing an spd will present.
3. No need for spd.
4. Yes an spd is required unless in the case of answer 2 in a domestic installation.
