Mek sure Geordies fridge with his 'broon' in has a multiple key lock Isolator and he has 'ALL' the keys.........simple really:):tounge_smile:

Lock Off Tag Out (or variations of the wording)

Record all lock outs with a permit and have then in a common location (dependent on the building) but the lock should ideally be capable of accepting multiple padlocks to prevent accidental energising of the circuit, clearly not possible with MCB locks!
 
Lot's of variations of safe isolation, which is also often attached to ''permit to work'' procedures. So you can go from the very basic procedure LOTO (a one man band affair) to the insane!! The most intense system i've ever worked with, was on the MTR (underground railway system) in Hong Kong, after and/or while track side was energised. A permit to work lock and tag from just about everyone and his dog was needed!! lol!!

The HK MTR is not a live rail system, but an overhead catenary system. Not saying that a safe permit system wasn't needed in these tunnels, far from it, but the system the Brit based H&S contract dept had going, was just insane!! Eventually when the MTR maintenance dept took the system over, got modified, with the removal of all the unnecessary dross. In fact with a system that was in place on the working 1st phase of the project...

I dare say they had a generic based on the London Underground system and others in Europe that use a live rail system, they would have been better adopting the track side safety procedures of the overhead trains by the sounds of that.

Not according to the seconded London Underground Rail Signalling Engineer, who was also the SRE, of that department. He was the main guy that had the majority of the run-ins with them. He could regularly be seen spitting feathers at these guy's (among others) at every site meeting and on almost a daily basis!! lol!!

Sounds about right, I know a few Guys who worked on the Jubilee line extension project and they bemoaned the consultants all the time for being over the top on some of the safety regs they enforced...
