Has anyone made an updated sheet for form filling or direct me to the old page

I do not think there are updated ones at the moment but the two threads that spring to mind are Useful information for form filling and DPN5 Tick sheet but they are relevant to the older forms.

Yh was the useful information sheet, and if I remember correctly u multiply by 0.95 to correct, thx fellow wire monkeys
