Not clicked on the cert but you need to fill out a Electrical Instllation Cert for a new circuit. You will need to speak to Napit as you may need to be registered to use certain Certs. NIC EIC have Certs of a different colour for non registered contractors

If you have installed a single new circuit, or any amount for that matter use an Electrical Installation Certificate.
