where to buy installation certificate

I doubt Smiths will have them you will need to go to a wholesaler like screwfix or tlc direct

I would hope that WHSmith don't sell pads of EICs!
If your local wholesaler haven't got any in stock they should be able to order them for you.

If you are buying the kewtech ones you need TC1 and TC5 for a complete EIC

the NIC (bless them) will sell pads (green colour and unbranded) to anyone and they are easier to use than the kewtec ones if only because you done need multiple books.
