Any one know where I can get a blank RA form there are plenty examples but all have them filled in with examples and none are specific to out trade anyone ??

Cheers Arch but I have that one as part of my RA policy it is more of a blank thing i need that I can complete relevant to the site, I may have to knock one up on excell

Depends on the level of detail you wish to go into, these are a couple of examples, but it is probably best to amend them to match how you undertake risk assessments.
One is a task list basic risk evaluation and the other is a specific task assessment (with methodology).
I probably got them from here in the first place.

I have a one made up on excel with risk assessment on one tab, method statement on another and the acceptance form on the third. Titles, numbers & dates transfer between all tabs so it saves a bit of time. If you pm me your email address I will send you a blank one plus a couple of filled in efforts so you can see the way it's put together.
