I have to install an earth electrode, which I have never done before. What are the limitations with regards to location? Can I install the rod inside the house under the floor and cut in an access panel? I have looked at the regs and can't find anything that says you can't.

The usual practice is external and accessable for inspection
The limitations would be common sense things,dont sink it where it will smash through the sewer pipe or gas or electric and such or where it would be a trip hazzard or get bashed

Your suggestion is ok,it would be accessable
The worrying thing about doing as you propose is that you would be piercing the damp membrane of the property
I would not fit a rod internelly personally

What's the best way to knock the thing in?
Is it a simple case of bashing with a hammer or this there a special tool?
I've got to do one myself soon. I am going to buy an inspection pit, they are thirty odd quid from the wholesalers and will make the finished item look really good.
 
ive seen a device that goes in your drill/breaker,that will fit over the rod, and then hammer it in. Lightning protection people use them.
