I've just acquired a Metrel Instaltest 61557 & with it came a bagful of test leads & other bits 'n' bobs.

Amongst it all there is a normal looking rubber 13A trailing socket but on closer inspection, I found a 4mm test lead socket in the hole where the cable would go through if it were being used as an extension cable.

When I opened it up I found that all the terminals inside - earth, live & neutral are all wired together & to the 4mm test lead socket.

Can anyone please tell me what this is for?

Many thanks for your help.

What you have there is a simple adaptor more normally seen in PAT testing to allow a continuity test to be carried out on extension leads, the more common form is the plug version. The socket version is only usually used where a BS1363 13A plug supplies some form of non-standard socket/s for which a special plug adaptor would otherwise be required.
