2357 NVQ - Witness Statements

I'm trying to get everything together to hand in my NVQ folder, my assessor isn't being very helpful with telling me what is acceptable as a witness statement.
Can I literally just reference which unit it is, which outcome it is, and then the section it refers to?

For example: Carolyn has complied with - Unit 311, Outcome 1, Section 1.1.

I just want to know what is acceptable, feels like I've hit a brick wall with no forward help.

Voice your concerns to your local City and Guilds office, they will act upon it, they don't like their approved centres not towing the line.
