Recessed - not if one of the accessory screw lugs is fixed. Surface metal clad - yes

There is no requierment if there is 1 fixed lug. Its good practice regardless what type of lug is on the box and i will always install one
 
It's a habit I developed years ago always fit them regs or not
 
I fit them, slightly shorter than the line, neutral and cpc so it earths the box and acts as a "safety brake" should the accessory be unscrewed and pulled away by a numpty.

Should it not be slightly longer so it is the last thing to receive stress aswell as the cpc
 
I always leave 1 of the cpcs very long, double it over and put it into the terminal. That way if it's one of those crap back boxes with the bent metal and screw when that fails the cpc will remain intact
