Sorry if i am being an idiot but do you just want a document showing the end user (customer) how to look after the system ?
 
No, I am looking for a template document for setting up a rolling annual maintenance service similar to an annual intruder alarm contract, where the client gets a visit every year to check the system over, for a set fee
