if you mean number of points served,as in on your test sheet. count a light inc. switch a 1 point. a socket ( double or single ) as 1 point, a FCU as 1 point.
 
no. because a switch is not a point of utilisation. although you should test through the switch for your R1+R2 and Zs.
 
As Tel says, try thinking of Items that use current. so a FCU feeding a single socket would be 1 point, a light with switch would be 1 point. Now a Cooker panel that has a socket outlet on it could be classed as 2 one for the cooker and one for the socket.
 
I think it's only there to give an indication of modifications to the installation since the last inspection also covers the inspectors backside if someone unsafely mods the installation just after an EICR is carried out and someone unjustly tries to pin the blame on the inspector
 
dont fill it in, its not needed. but if you have done a rewire, then i would, so you can tell if DIY charlie has added a bit, it all goes pete tong, and he tries to blame you, you can tell him to off, and his guarantee is null and void.
