Risk Assessment - would like some help, please!

Hi,

I have been scouring the Internet, but to no avail to get hold of a “Risk Assessment Form” as a guideline to follow. Could someone let me have a copy of it that they have done at work or for their assessment.

I posted mine on this site sometime back. Can't remember exactly when but its here somewhere.

As I recall the elecsa one is an A4 sheet of risk situations like lifting floorboards,moving furinture.You are supposed to circle the ones you are going to be doing and hand it to the customer.Must start using it myself if I can find it.

Doesn't mention anything about risks associated with electricity.
